Phelps Memorial Home Health Care and DME can assist with recovery and
rehabilitation in ones own home. This home care can be an economical and
caring alternative to hospital and
 nursing
home stays. The Phelps Memorial professional staff evaluates each
patient's needs and sets up an individualized plan of home care.
Office hours are Monday through Friday 8:00 a.m. to
5:00 p.m. An RN is on call 7 days a week, and 24 hours a day. Please

and ask to have a home health nurse paged.

Services Available
• Skilled Nursing services provide nursing
care to assist in recovery and rehabilitation
in your own home. This may include IV
therapy, wound care, teaching of specific
conditions or any other skilled need.
• Home Health Aide services provide
assisted living care, including bathing,
personal care and assist with exercise as
ordered.
• Physical Therapy services improve mobility,
strength and endurance.
• Speech Therapy services provide
rehabilitative services for speech, hearing
and language disorders.
• Occupational Therapy services provide
rehabilitation and aid to daily living skills.
• MSW-Services provide assistance for
patients social, emotional, or economic situation. |
