The Patient Registration Department of Phelps Memorial Health Center
welcomes you as a patient. We are pleased that you have chosen to
receive medical services from our facility. We are committed to making
your hospital visit as comfortable and convenient as possible.
The admissions department is located at the main entrance on the east
side of the building at 1215 Tibbals Street.
The main entrance doors are open:
Monday through Friday from 5:45 am until 9:00 pm
Saturday & Sunday from 7:00 am until 9:00 pm
If you arrive at the hospital prior to 5:45am, we ask that you use the
ER entrance on the north side of the building.
All services require you to stop in the Admissions Department. Upon
arrival you will be asked to verify all demographic information and
provide us with your insurance cards.

Insurance and Pre-certification
To ensure your peace of mind while you are hospitalized, we suggest that
prior to your admission you be aware of the type of coverage your
insurance policy provides. It is your responsibility to determine if
pre-certification is required. You will need to bring your insurance
cards and your pre-certification number with you when you come to the
hospital. Failure to obtain authorization may result in reduced
insurance benefits from your insurance company, therefore raising the
amount due by the patient.
Advance Directives
If you have any advance directives
such as a living will, or durable power of attorney for health care,
please bring a copy with you. This directive will be used as a guide for
the decision-making of all care givers should you become unable to
communicate your desires about medical care. If you do not have a living
will or durable power of attorney information can be obtained from us at
the time of admission.
Regarding your bill
Phelps Memorial Health Center contracts with several physicians and
groups to provide professional services to patients. In addition to
receiving a bill for services provided at Phelps Memorial Health Center,
you will be billed a professional fee separately if you receive services
provided by:

Radiology ( CT, MRI, Ultrasound, etc.)
