Outpatient Diabetes Self-Management Program
Diabetes is one of the most severe health problems of our time. Our education program is taught by a Certified Diabetes Care and Education Specialist (CDCES) and registered dietician to help give you insights to effectively manage your diabetes on a day-to-day basis so you can live a healthier life. After completing the program, participants have experienced reduced hemoglobin A1c levels and are more confidence in their ability to manage their diabetes.
What You Will Learn
This multi-part course is offered at various times to accommodate individual schedules. Family members and significant others are welcome to attend at no additional cost. For those with special needs, we offer personalized instruction as needed. The charge for this program is billed directly to your medical insurance and is covered by many plans. Financial assistance is available for those with no payment source.

Our Diabetes Education Program is in conjunction with Bryan Health in Lincoln, NE, which is recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.
Additional information and education is available to fine-tune diabetes management using Continuous Glucose Monitoring (CGM), insulin to carb ratios, and insulin pumps.
